What this document covers
The registration form for an online nutrition consultation asks for information we cannot infer on your behalf: your name, how to reach you, the topic you want help with, what you and your household usually eat, and any health context you choose to share. Because that information shapes the advice you receive, the way it is stored, reviewed and eventually removed is set out here in full rather than summarised in a banner.
Eight sections follow, in the order most visitors ask about them. If you only need one answer — how to have your record corrected or deleted — the section on your rights carries the phone number and the steps. If you are comparing us with another service before registering, the sections on what we collect and why are the ones worth reading first.
Section one
Data we collect
When you submit the registration form, we collect the information you enter. In practice that usually means your name and contact details, the consultation topic you selected, your meal and activity information, and any health context you choose to provide. Completing a section is not compulsory — a shorter description of your routine still gives the nutritionist something to work with, though it may mean the first session leans more on questions than on review.
Alongside the form, we collect basic technical data generated by your visit, such as your browser type, the pages you viewed and the approximate time of the request. This is ordinary server logging. It records that a page was opened, not what you typed into it.
Section two
Why we collect it
Registration data is used to review your request, match you with a suitable nutritionist, schedule your session, and deliver the consultation and any follow-up you agree to. The nutritionist needs your meal and activity information before the call, otherwise the first half of the session is spent gathering what a form could have collected in five minutes.
Technical data keeps the site functioning and helps us understand general usage patterns — which pages visitors open, where a form is abandoned, whether a page loads slowly. We do not use consultation information for advertising purposes. Your health context is not passed to an advertising audience, and it is not used to build a marketing profile of you.
Section three
Cookies and similar technologies
This site uses cookies for basic functionality and, where applicable, for analytics. Necessary cookies keep the session and the form working; analytics cookies help us see which parts of the site are actually used. Details on the types of cookies used and how to control them are set out in the cookie policy, which also explains what changes if you block them.

Sharing and disclosure
Consultation information is accessible to the nutritionist handling your case and to staff who arrange scheduling. Those are the people who need it to do their part of the work. We do not sell personal data.
Scheduling staff see your name, contact details and appointment time. They do not need your meal notes or health context to book a session, and access is arranged so they can work without opening it.
We may disclose information where required by Indonesian law or by a valid legal request. Where a request is narrow, we share only what the request covers.
If you would rather discuss a case without putting it in writing, phone the office and we will note the request on your record instead. The number is on this page.

Section five
Retention
Registration and consultation records are kept for as long as needed to deliver the service and to meet administrative and legal obligations. For a single consultation that usually means the record stays available while follow-up is possible; longer if a legal or accounting rule requires it.
When records are no longer needed, they are deleted or anonymized. Anonymized records can no longer be linked back to you, which is why they may remain in general usage statistics after your identifiable record has been removed.

Section six
Your rights
You may request access to the personal data we hold about you, ask for corrections, or request deletion where no legal obligation requires us to keep it. Requests can be made by phone at +62 711 356 789 or through the contact details published on this site.
If you ask for correction, tell us which detail is wrong and what it should say. If you ask for deletion, we will confirm what can be removed and note anything we are required to keep. We answer by phone during office hours, and we do not charge for these requests.
Ask what we hold
We confirm which records exist under your name and what they contain, apart from notes a nutritionist keeps as a private working draft of professional advice.
Correct what is wrong
Contact details change and meal notes go stale. Tell us the correct version and we update the record rather than opening a new one beside it.
Ask us to delete
We remove what we are not obliged to keep. If a legal or administrative rule applies to part of the record, we tell you which part and why.
Section seven
Security
We apply reasonable technical and organizational measures to protect the data you submit: access limited to the people who need it, records kept in systems we control, and staff who handle consultations working inside those systems rather than in personal copies. The contact details on this page are the ones to use if you want to check whether a message claiming to be from us is genuine.
No method of transmission or storage is fully secure, and we cannot promise absolute protection. If a problem is discovered that affects your record, we tell the people affected.
Two practical habits help on your side as well. Send consultation material through the form on this site rather than by ordinary message on a shared device, and close the browser tab when you have finished. Neither is complicated, and together they remove the most common way information travels further than intended.
If you believe your data has been handled in a way this policy does not describe, phone the office and ask for the person responsible for records. We will look at the case and respond with what we find.
Section eight
Changes to this policy
This policy may be updated to reflect changes in our practices or in applicable law. The current version is always the one published on this page, so there is no separate archive to check and no notification to wait for. If a change affects how consultation information is handled, we will make that clear in the text rather than leaving it in a footnote.
Questions about this policy, about a specific record, or about how the registration form is processed can go to the same office as everything else. Calls are answered Monday to Friday, 09:00–18:00.

Talk to the office before you register
If something in this policy is unclear, or you want to know how a particular detail in your own record would be handled, call the office during working hours. Staff can answer questions about the registration process, about scheduling, and about what the site stores, without you submitting anything first.
